If you have a patient handout, you can find it on the portal by clicking "Review Medical Record", then on the left hand side of the screen, select "Office Visits" from the menu. On the date of your visit, you will see a column called "Documents". Click the name of the document and you will be able to view the PDF or download it.

If we upload a document to the patient portal for you, here is how to find it:
To find it, from the home screen of the Patient Portal click the box that says "Review Medical Record". On the left hand side of the next screen, click the tab called "Documents" then click the drop down option that says "Patient Documents". You will see a PDF attachment
Lab Orders
We work with Quest Diagnostics for labs. Lab orders are sent automatically to Quest Diagnostics. If you want to have your labs done elsewhere, bring a copy of your lab orders to the lab of your choice. You will be responsible for ensuring the lab you choose gets the results to us.
If labs were ordered for you during your visit, please schedule yourself for blood tests for labs at Quest Diagnostics. Plan to bring yourself, your ID, and your insurance card to your lab appointment. Please be sure to check in or register with whatever name is associated with your health insurance.
Please get labs done fasting, meaning nothing to eat or drink except water or medications for 8-12 hours, unless you were instructed to get non-fasting labs. This is easiest done if you avoid eating after midnight and get labs done before breakfast. This will allow me to get accurate cholesterol and blood sugar testing.
Please also check your supplements and stop any that contain biotin 3 days before lab work. Biotin interferes with many blood test results. Biotin can be found in multivitamins, b-complex vitamins, and "hair skin and nails" vitamins.
Also if you are having hormone labs drawn, it takes 10-14 days for testosterone results to come back, so please ensure you have your blood drawn done with enough time for your results to come back before your follow up appointment. If you are taking hormone injections, please get labs drawn 3-4 days after an injection.

As a reminder, it is your responsibility to understand your insurance coverage of any lab tests ordered during your visit.
To find your orders on the patient portal, log in to the patient portal. Click the box that says "Review Medical Record". On the lefthand side of the the screen, click the box that says "documents" then click the drop down option that says "Patient documents". You will see a PDF likely called something like "Lab Orders". Click it and you will be able to view and download a copy.
We recommend having a copy of your lab orders available when you get your blood drawn in case the phlebotomist can't find your orders.
Referrals or Imaging
We work with many radiology clinics and specialists offices.
If you had a referral made or imaging order placed, you will get a portal message within the next week from someone on our admin team with the contact information for your referral. Generally, we recommend planning to contact the clinic you were referred to in about a week and contacting that clinic weekly until you are scheduled. Before messaging us to let us know the clinic didn't receive your referral, we recommend asking them what date they are processing referrals from to see if they are just behind.
