Here are the steps:
We will provide you with information about our practice, policies, and if it is a good fit, we will provide you with new patient paperwork via dropbox sign to complete electronically.
Once the paperwork is completed, you will be registered in our electronic health record.
Once you are registered, you will be scheduled for a 60 min new patient appointment for primary care or a 90 minute new patient appointment for mental health. All new patient appointments must be in person.
You will be granted access to the patient portal once you have been scheduled. Visit our patient portal to message your provider, pay your bill, schedule follow up appointments, request refills, and complete telehealth visits.
We will verify your insurance is active, but it is your responsibility to determine if the provider you are scheduled with is in network with your specific plan.
