At Lavender Spectrum Health, we are committed to providing compassionate and inclusive care. As a small, independent practice, our staffing and financial model are different from large hospital systems, which affects how we are able to provide certain accommodations.
We use a secure patient portal and electronic forms for most communication and document sharing.
Our administrative team works remotely, so we do not have staff available on-site to complete forms or to provide ongoing phone-based communication.
Because our providers spend more time with patients in appointments than most clinics, our administrative capacity is limited compared to larger organizations.
If you experience difficulty with electronic forms or portal communication, please let us know so we can discuss reasonable options. Depending on your needs and situation, this may include:
Using assistive technology such as free screen-reader software to access written materials.
With a valid release of information or power of attorney document, you can designate a trusted support person to help you complete forms or use the portal, as long as that person is available to help you access communications on the portal which may be time sensitive from your provider
Having limited assistance incorporated into a scheduled clinical visit, when medically relevant.
Receiving written instructions or educational materials in accessible electronic formats.
If you have a disability that prevents or substantially limits your ability to communicate with us in the format provided by our office, please contact us to let us know so we can attempt to help you.
If your communication needs require regular in-person clerical assistance (such as someone completing forms for you on-site or communicating routinely by phone), we may not have the staffing capacity to provide that level of support. In those situations, we are glad to help connect you with larger health systems that have dedicated accessibility staff and greater administrative resources.
No On-Site Vaccinations or Phlebotomy
We are not able to offer on-site vaccinations or phlebotomy. Instead, we will refer you to the appropriate clinic to receive vaccinations or blood draws.
Our goal is always to work collaboratively with you to find the best possible solutions within our resources while ensuring you receive safe, respectful, and high-quality care.
Lavender Spectrum Health does not discriminate on the basis of disability in our health programs and activities. In keeping with the Americans with Disabilities Act (ADA), Section 504 of the Rehabilitation Act, and Section 1557 of the Affordable Care Act, we strive to ensure effective communication and reasonable modifications for patients with disabilities.
Our goal is to make our website and electronic forms usable with assistive technologies such as screen readers, magnifiers, and speech-to-text tools, and we are working toward conformance with WCAG 2.1 Level AA. If you have difficulty accessing any part of our digital tools or need an accommodation to communicate with your care team, please let us know so we can work together on an accessible solution.
If you experience any barriers accessing our website, patient portal, or electronic forms, please contact us at (360) 727-1641 or info@lavenderspectrumhealth.com so we can work with you to provide the information or services you need in an accessible way.
