At Lavender Spectrum Health, we are a small independent outpatient practice designed to provide thoughtful, relationship-based medical care with longer visits and individualized attention. Many patients value our boutique model because it allows for personalized care, continuity, and a more direct provider relationship.
At the same time, our practice model is different from large hospital systems or federally funded health centers. We do not have embedded on-site social workers, full-time case managers, or an internal network of specialists. We are also not able to provide intensive ongoing care navigation that requires repeated coordination across multiple outside organizations, insurers, hospitals, housing systems, disability systems, or community agencies.
We are often a strong fit for patients seeking:
Primary care in a smaller, relationship-based setting
Gender-affirming hormone therapy and LGBTQ+ inclusive care
Preventive care and chronic disease management
Behavioral health medication management within scope
Longer visits for complex discussions
Some patients are best served in a larger integrated health system with embedded support services. This may include patients who need:
Ongoing case management or social work support
Frequent coordination among many specialists
Intensive insurance authorization support
Complex disability / housing / benefits navigation
Wraparound behavioral health services
Hospital-based multidisciplinary programs
Internal referral networks with many in-house specialists
Examples of larger systems may include Oregon Health & Science University, Legacy Health, Kaiser Permanente, community health centers such as SeaMar, or other integrated medical groups depending on insurance and location.
Healthcare needs can evolve over time. If your care needs grow to require a level of case management, social work support, behavioral health integration, or multidisciplinary coordination beyond what our practice model can reasonably provide, we will discuss this with you openly and respectfully.
When appropriate, we will help facilitate a transition of care to a clinic or health system better equipped for those needs. This may include providing referral suggestions, reasonable transition support, and transferring medical records upon request so that care remains as smooth and continuous as possible.
We strive to offer timely access and may offer same-day appointments when openings are available. However, we do not reserve dedicated same-day appointment slots. If no appointments are available and your concern cannot wait, you may need to seek care through urgent care or the emergency department depending on severity.
Portal messages are first reviewed and triaged by our nursing team. Messages are escalated to a provider when clinically indicated. Please allow up to 72 business hours for portal message responses. Portal messaging should not be used for urgent or emergent concerns.
If you are specifically seeking guaranteed same-day availability, immediate direct access to your provider, or highly responsive on-demand communication, a concierge medicine or direct primary care model may be a better fit for your preferences and needs.
We believe patients deserve transparency about what a clinic can and cannot realistically provide. If we believe another setting would better meet your needs, we will tell you directly and respectfully. Our goal is always to help patients receive care in the environment most likely to support their health.
