Your expected patient responsibility (EPR) will be due at the time of your appointment. Any payment collected will be applied toward your copayment, coinsurance, deductible, or other patient responsibility as determined by your insurance plan.

If you have an in-office visit, please avoid carrying strong scents such as smoke, perfume, fabric softener, deodorant, or detergent to help maintain a sensory-safe space for all patients.
Please arrive 30-minutes before your appointment start time to complete check-in, pay your balance, and ensure your insurance is active. If you arrive 10 minutes late or more, you will be marked as a no-show, and services will not be provided. Front desk hours are 9:30 am - 5:30 pm, Monday - Friday. If your appointment is outside of those hours, please be sure to complete your virtual check-in on your patient portal before your visit.Β
To help maximize your time with your provider, please check in virtually on the day of your appointment using the patient portal.
Be sure to complete the pre-visit health screenings.Β
Check-in sections can be skipped if theyβre not relevant to your visit.

Anyone accompanying you must also wear a mask, and abide by our Guest and Visitor Code of Conduct
If you are coughing or visibly ill, your provider may ask you to either complete your visit from your vehicle (if appropriate) or reschedule to protect other patients and staff
24-hour notice is required to cancel or reschedule
A fee will apply for late cancellations or no-shows
If you arrive more than 10 minutes late, your provider may not be able to see you
Three no-shows or late cancellations within 1 year may result in dismissal from the practice
Balances unpaid for more than 120 days may be sent to collections, resulting in discharge
Please ensure any outstanding balances are paid before or shortly after your appointment.

You must request to change your appointment to an e-visit at least 2 business hours (9:30 am - 5:30 pm, M-F) before your appointment starts. This allows the administrative team enough time to see your message and approve the adjustment.
