Your expected patient responsibility (EPR) will be due at the time of your appointment. Any payment collected will be applied toward your copayment, coinsurance, deductible, or other patient responsibility as determined by your insurance plan.

1. Log In Early
Please log in to the patient portal 20 minutes before your appointment at:

This gives you enough time to complete the required check-in steps before your provider joins.
2. Complete the Check-In Process
Click on “Appointment Check-In” once logged in.
Pay your balance or EPR.
Follow the on-screen prompts and complete any relevant forms.
Be sure to complete the pre-visit health screenings.
Check-in sections can be skipped if they’re not relevant to your visit.
3. Start the Video Call
Once check-in is complete, a video window will appear. This means your telehealth session is ready to begin—your provider will join you shortly.
4. Technical Tips for a Successful Visit
Use a strong internet connection. Wi-Fi is preferred over cellular data.
Allow camera and microphone access in your browser when prompted.
Find a quiet, private location where you can speak freely and without interruption.
Your provider can address up to two health concerns per visit.
24-hour notice is required to cancel or reschedule appointments.
A fee applies for no-shows or late cancellations.
If you are 10 minutes late or more, the appointment may be marked as a no-show.
Three no-shows or late cancellations within a year may result in discharge from the practice.
Unpaid balances over 120 days may result in discharge and referral to collections.
It is your responsibility to verify that your insurance plan covers telehealth visits.
Patient Code of Conduct
To avoid disruption in care, please ensure any outstanding balances are paid before your visit.
