Navigate to the patient portal here: http://yourhealthfile.com/ 

If you have a patient handout, you can find it on the portal by clicking "Review Medical Record", then on the left hand side of the screen, select "Office Visits" from the menu. On the date of your visit, you will see a column called "Documents". Click the name of the document and you will be able to view the PDF or download it. 

If we upload a document to the patient portal for you, you can find it by navigating to the home screen of the Patient Portal. Click the box that says "Review Medical Record". On the left hand side of the next screen, click the tab called "Documents" then click the drop down option that says "Patient Documents". You will see a PDF attachment
