Your expected patient responsibility (EPR) will be due at the time of your appointment. Any payment collected will be applied toward your copayment, coinsurance, deductible, or other patient responsibility as determined by your insurance plan.
Patients are responsible for understanding their own insurance benefits (please review our Insurance Policies)

Please avoid carrying strong scents such as smoke, perfume, fabric softener, deodorant, or detergent to help maintain a sensory-safe space for all patients.
Please arrive 30-minutes before your appointment start time to complete check-in, pay your balance, and ensure your insurance is active. If you arrive 10 minutes late or more, you will be marked as a no-show, and services will not be provided. Front desk hours are 9:30 am - 5:30 pm, Monday - Friday. If your appointment is outside of those hours, please be sure to complete your virtual check-in on your patient portal before your visit.

Masking: Please wear a surgical or better mask (KN95, N95, etc.).
Anyone accompanying you must also wear a mask, and abide by our Guest and Visitor Code of Conduct

24-hour notice is required for any cancellations or rescheduling
A late cancellation or no-show fee will apply if you cancel with less than 24 hours' notice or miss your appointment
Three late cancellations or no-shows in a 12-month period may result in discharge from the practice
Any balances unpaid for 120+ days may be sent to collections, resulting in patient discharge
To avoid disruption in care, please ensure any outstanding balances are paid before your visit.
